Evaluation: It could be many things from an infection from a tooth to a salivary stone to other pathologies. You will need to go to the doctors for a definitive diagnosis.

If it is: In the skin, maybe deep acne. If it is deeper it could be stones in the parotid gland, or even a tumor. I don't think it is related to your thyroid condition or hay fever. Have it checked out.
