THyroid issues: Thyroiditis means inflammation of the thyroid and the chronicity simply means the length. Possibly it means you have had a long term inflamed thyroid that went undiagnosed.

Autoimmune likely: Chronic thyroiditis usually refers to a chronic problem of the thyroid (also called chronic lymphocytic thyroiditis, or hashimoto's thyroiditis) in which there is inflammation of the thyroid caused by the immune system attacking the thyroid (autoimmune). Many people become hypothyroid, but it can take time and initially present as an enlarged or nodular thyroid., with normal thyroid levels.
