Go to ER ASAP: Severe pain in the upper back and chest pain on breathing requires immediate attention. Several serious conditions can cause these symptoms, including pulmonary embolism(blood clot to the lung). Other less serious causes include a viral pleurisy and herniated thoracic disk, among others. Assuming this pain just started, and hasn't been present for weeks, you should go to the ER for evaluation.
