Fibromyalgia: The defining characteristic is whether the pain is strictly in joints or is it in the meat of the major muscles of the lower back, legs and shoulders. Pain in muscles and with muscle use suggests fibromyalgia. Pain in the joints suggests a rheumatologic problem. Lyme disease, lupus, and immunological disorders complicate the diagnosis.
