Several causes: The ear fullness is either referred from the maxillary sinuses, inflammation of the nasopharynx from the drainage or infection, or swelling of the eustachian tubes. The headache is either from transient blood pressure increases during the cough causing swelling of the sinus linings or temporary distension of the veins inside the skull which are very sensitive (cough headache).
