More info?: Did you injure yourself? How long does it last? Anything make it better? Anything make it worse? Does food affect the pain? Have you had any surgery ? How long has it been there? Have you tried any medications? Did they work? Any nausea? Fevers? If this is recent you could try ibuprofen, heat, massage. If long standing or worsening see a doctor to begin workup.

You need to see a MD: It is not normal for a 49 year old to have constant pain in their upper back. Without injury it is probable muscle spasm particularly of levator scapulae and/or the rhomboids. If so, and confirmed by an md, then a massage therapist or pt can help. I like to have them do graston technique...

Scapula Pain: Inflammation can occur underneath the shoulder blade where there is a bursa. This is called scapulothoracic bursitis and can be treated with therapy, NSAIDS, pain medications, and possibly a steroid injection. Neck or shoulder: This type of pain can be from the neck or the solder. Pathology in either area feel the same to the patient. If you have more shoulder pain, then she a shoulder doc or sports med doc. If you have more neck to the intrascapular region, consider seeing a spinal specialist.
