Physical exam: Peritonitis is inflammation of the peritoneum or lining of the abdominal cavity. It develops secondary to an infectious process somewhere else (usually) such as appendicitis, and manifests as exquisite pain directly on the site of inflammation (i.e. Right lower quadrant of abdomen). Appendicitis is appendicitis, and needs prompt treatment (removal).

Fever, rigid muscles: Pain on walking are some of the signs. Elevated white blood cell count, low blood pressure, rapid heart rate are other signs, but these are signs only a doctor can tell. If you suspect appendicitis, you need to go to nearest hospital immediately. Nausea, vomiting, right lower abdominal pain are signs. Infection causes inflammation, so here it means same thing.
