Prognosis: Prognosis depends on the total picture of the patient's health status, cardiopulmonary status, liver and kidney function. Should discuss with her doc. Sounds like some increased risk to the surgery but maybe necessary with risk benefit consideration.

TIA's in a female: You mother should be seen by a physician in the hospital after she has her carotid endarectomy. She may have arterial disease of the heart and her legs, in the hosptial, her coronary circulation can be evaluated by non-invasive tests and if she has disease there, preventative measures can be done. Tia's and strokes are signs of generalized atherosclerotic disease of her arterial system. Good luck.

Difficult challenge: The carotid procedure would usually have a low complication risk, of 1-2% in the hands of an experienced vascular surgeon, but her situation is highly complicated. Agree that internal medicine close followup is critical pre and post-op.
