No: Shouldn't be dangerous. Much safer than not wearing it in case of accident. Now if case you have absent chest wall segment or some sort of weakness then consult with your doctor.

Unlikely: I am not aware that wearing a seatbelt shoulder strap is particularly dangerous for any type of heart defect, but without knowing your exact type of heart defect, i cannot answer your question unequivocally.

No: The shoulder strap is designed to add the inflated lungs as a cushion to help disperse the tremendous forces otherwise placed on the lap belt. Heart defects make you no more or less in need of this safety feature.
