Why are some doctors often reluctant to do simple biopsies?

Difficult question: Perhaps the physician is in experienced or afraid of causing a hemorrhage or the tissue is so infected they planned to wait there are many reasons however a skilled surgeon would biopsy as necessary i recommend a second consult.
