IV insulin: A diabetic whose glu is 1000 is usually someone who is very ill and in the hospital, and this person will likely get IV insulin. Dextrose is glucose, and would not be given. However, once the Insulin lowers the glucose to about 150-200 the hospital doctors may want to continue the Insulin but then give extra glucose with the Insulin to reverse the abnormal metabolic condition, the acidosis.
