Needs biopsy: It may be sarcoidosis, or carcinoma with metastasis in liver and lymph nodes and in my opinion you may need biopsy to make the diagnosis.Ask your pulmonologist or internist what is his/her opinion as they have access to the films and your history.

Depends : There are some liver lesions of that size that are benign. There are some lung lesions of that size that are benign. There are lymph nodes that are benign even much larger. So, additional exams to clarify. Pet scan may show activity and suggest cancer or inflammation. Ultimately may need biopsy of one of the spots. Mediastinoscopy for the nodes is well tolerated. The other two may be too small

Follow up CT: The importance of the findings may depend on why your ct scan was done. Current guidelines suggest Incidental single lung nodules between 4-6 mm in patients over 35 need follow ct scan, the interval between scans is dependent on the risk of cancer (smokers, exposure to asbestos, etc). 10mm(1cm) lymph note near trachea is upper limits of normal size.
