Would be unlikely: Tracheostomy sites do not usually spasm. The tracheostomy site could be gradually scarring down, and this could explain why insertion of a new tube hurts. Make sure you are replacing it with the same size and manufacturerer's tube. Make sure it is well-lubricated. If there is still pain, see your doctor.

It depends: Tracheal granulomas are not typically painful. However, certain types of granulomas can produce pain. Tracheal granulomas, depending upon size and location, can cause airway obstruction and respiratory distress, and thereby discomfort. If a tracheostomy was difficult to replace, that could produce discomfort as well. Stoma do not typically spasm, but can narrow if a trach is out for a long time.
