Grandmother: You should have her evaluated. Symptoms described may be due to a zenkers diverticulum at the upper esophagus where food can get stuck causing bad breath and inability to eat well.

Needs a medical eval: These can be symptoms of a serious medical condition that is not diagnosed. I recommend your grandmother see her primary care physician for a thorough evaluation and blood work to rule out a possible serious disease.
