Suggest arthritis: No trauma. Sleeping with it bent would be a very unlikely cause. Does it respond to Naproxen or ibuprofen? The fact that you may have less pain during the day, but still have pain suggest you may have mild arthritis in the joint. If you were my patient, after a good hx and physical, i would probably inject with cortisone and xylocaine, (lidocaine) to see if your pain improves!
