Why swelling?: Need to decide why swollen there can be a variety of cause and not all are due to sle.

Gradually, yes..: Recurrent vasoconstrictive episodes like in raynaud's phenomenon can lead to atrophic or ulcerated fingers/toes and surrounding swelling. Some of these changes end up being permanent over time.
