Neuropathy: It could be a sign of neuropathy. There are many other causes of neuropathy other than diabetes. I would recommend you see a neurologist for appropriate work up and evaluation.

Consider Spine: This apparent posture related sensation in your toes/soles when you lie down may be due to some form of pressure on the nerve roots at your lumbar spine. These nerves branch out and run all the way down to the tips of your toes. Consult a doctor for light exercise and stretching movements that you can do to improve core strength and flexibility. If you have a "beer-gut" you need to lose it.
