Start w/family doc: As a general rule, go see your family physician (or general internist) for most of your concerns. S/he can diagnose & treat most conditions. If warranted, s/he can then refer you to the appropriate specialist. In the case of bumps behind testicles, if s/he can't make diagnosis, you might be referred to urologist.

Your regular doc : Your usual internist or family physician may be able to diagnose what you have (possible cyst in the epididymis, varicose veins, cysts in the skin, etc). If not, you may be referred to a urologist for an exam. If the lump is actually within the testicle, you should see a urologist immediately.
