See orthopedic MD: Swelling of the ankle associated with a sensation of a "muscle pull" can represent a rupture of one of the ankle tendons or tendonitis. See a doc to get examined . They may require a MRI to diagnose it and possible surgery. Otherwise swelling of the ankle alone (just on one side) may represent arthritis. Swelling of both ankle may indicate you retaining fluid.

Possible fracture: Did you twist or injure your ankle before the swelling began. You could have a severe sprain, an avulsion fracture or even a stress fracture. Ankle injuries are notoriously slow healing especially if not treated correctly. See your doc.
