Wait at a few days: Low back problems shouldn't cause shoulder pain. If you weren't injured then wait at least a few days, make yourself comfortable, use ice or heat and give your pcp a call. It is not likely to be an emergency unless it gets worse. The er isn't ideal because you will not see spine specialists and it is very expensive to receive care in that manner. Always call your pcp first.

Hard to say: There are many potential causes, so it is hard to say. Cardiac problems, perforated ulcers, and a host of potentially life threatening conditions can cause those symptoms and so consider being checked. Probably the most likely cause in someone who has had two prior back surgeries is you may have developed a disc problem in your mid back or neck which can cause pain radiating into the shoulders.
