Could be ovary: While uncommon, we worry in women with severe pelvic pain about a twist of the ovary, called tortion, which can cut off that ovary's blood supply. If this happens sometimes the ovary has to be removed. Other possibilities include pain from ectopic pregnancy, ovulation bleeding, appendicitis, tubal/ovary infection, diverticulitis, etc. Best to see your doctor to narrow this down.
