Yes: Sinus infections can be invisible in plain x-rays. Symptoms are such that x-rays are often not needed unless routine treatment fails or infection recurs.

Yes: Sinus x-rays have poor sensitivity for the detection of sinusitis. Most of the time, the diagnosis is based on the symptoms and clinical examination. In complicated cases, ct scan can be helpful.
