Small fibre : When cutaneous small fibre nerves are afflicted, the patient loses pin and temperature sensation, but may develop widespread varying levels of pain, and punch biopsy shows low fibre density. Not uncommon in diabetes, may be seen in immune neuropathies, and likely associated with porphyria, and alcohol problems. Metanx may help, ivig in some. Prognosis mixed. May need neuromusclar specialist.
