Possibly: Also worth seeing a physician to determine if they have another alignment issue such as flat feet or misalignment of their hips. This all could contribute to their appearance of knock knees and pain.

Usually not: Many children are knock kneed, or have genu valgus. As long as the angle is small, it usually does not cause pain. On the other hand, if there is a history of trauma, or infection, and the deformity is not symmetric, or mild, it may cause a significant force imbalance. This can lead to issues with the ligaments and cartilage with resultant pain.
