Several: There could be several causes including such conditons as diverticulitis or an ovarian issue to name a few. Any "causes over the computer though would be a guess. See a dr. For a clinical exam and some testing to determine the true cause and then your treatment can start. Good luck.

Not related: This is unlikely to be related to your appendix if it was completely removed. This is more likely related to your ovaries or other pelvic organ. See a doctor get checked out.
