Depends: If the nail can be removed cleanly, whether partially or fully, you can try- in a clean-as-possible setting and apply some antibiotic cream and a non-adhering dressing, like telfa. If you aren't sure- a podiatrist can do it safely and antiseptically.

Nail injury : The answer to this question is that it is probably better if you have the nail avulsed. The reason being is that the nail may have sharp edges which can catch on a sock or shoes which could cause more trauma to the area. A nail aviation is a simple procedure and most podiatrists and family physicians can perform this procedure in the office with no down time.

No: I generally don't recommend home surgery. Clean it thoroughly and bandage it with an antibiotic ointment, gauze and paper tape. Change the dressing daily. If it shows signs of the nail loosening, or odor, pain or drainage which might indicate infection, see a podiatrist and have it evaluated.

You need to see: A foot doctor. It would be wise to get an x-ray to see if you fractured your toe. If the nail bed was lacerated in any way, you should be on an antibiotic. If that is the case, your tetanus history should be reviewed as well.
