They are following: A five week vaginal ultrasound can be misleading and even in twin pregnancies, frequently one twin will vanish. They may be following the development with serial ultrasound prior to confirming that this is, in fact, a twin pregnancy.

Not sure: Can't explain why someone would or wouldn't without being there. Maybe it wasn't as clear cut, maybe they wanted a followup. Maybe they didn't know....
