A few possibilities: Stabbing pain is not good. Appendicitis may cause bowels to act differently (no bm). Bladder problems would be rare in a little boy this age, but should be checked out with a urinalysis. Although he is a little out of the age-range, look up intussusception. You may need to take him in for an evaluation. The the most important details will be in your serial abdominal exams.
