Pain Will guide you: If you cannot live with the pain in your current condition or you have weakness, then surgery may be the best option for you. Most microdiskectomies are very successful- but don't go into any surgery without carefully considering it. Many people get over a herniated disk in 3-4 months without surgery. Don't let yourself get too bad before you do something. Talk to your surgeon- he will help you.

Try not to: It all depends on how back the nerve involvement is, and how much you can tolerate pain/other symptoms in terms of quality of life. I recommend you try all non-surgical treatments first unless there is a neurological emergency (full foot drop for example). There are multiple types of surgery, and you are advised to lean to the least invasive one if at all possible. Get two opinions (or more).
