Palsy : A superior oblique palsy can cause this. The most common cause of a palsy is a micro vascular occlusion from diabetes or hypertension. Glasses and large difference in prescription between the two eyes may cause double also when looking down through the reading portion. If micro vascular it usually resolves within 3 months.
