Ice and topical crea: Ice can give immediate topical relief. Aspercreme will penetrate about 3-4cm and if you are not allergic to Aspirin this will deliver some anti inflammatory medicine directly to the area. After icing you could also try a counter-irritant topical cream. In the morning, you should talk with your dr's office about pain meds to have on hand for flareups.

Ice: I don't know how long you've had a shoulder problem bit it sounds like for a while so a warm compress might help to get you through the night... If you re-injured the shoulder follow "rice" rest, ice, compress and elevate. Hope u get better.
