A subungual melanoma for someone who is 33 is exceedingly rare, and i would be surprised if the result is positive. Charges by dermatologists vary by state and by city, depending on their costs. Usually <$200 for a biopsy, plus about $75-125 to the pathologist to interpret the biopsy.

Subungal melanoma : Biopsy does require lifting the nail to biopsy the area. But local numbing is used so you don't feel that.

Shouldn't be: I agree with the other doctors. More importantly get it done if suspicion is high for melanoma. It is not worth avoiding it when catching and treating it early could save your life.
