In the 20% of those who don't know they have hiv, is it because they don't get symptoms of early hiv/ars or because they don't associate them with hiv?

Both: Both...but many people (up to 70%) who do not get symptoms at sero-conversiom can go until very late in the disease with little to no issues and then present at end stage with issues.
