Go to hospital: Go to the hospital to have the wound cleaned and started on antibiotics. You will need to have your tetanus immunization updated and if possible check to make sure the dog does not have rabies.

Go to ER: I would first thouroughly wash this for at least 10 minute under clean running water. This is a potentially serious problem and i typically recommend one dose of IV antibiotics especially for penetrating wounds to minimize the possibility of an infected complicated wound.
