Upon completion of medical school and residency, he started a private practice in internal medicine in the northshore Chicago area. Meds/lifestyle: Gout is caused by excessive uric acid crystals in your body. This can be reduced by diet low in purine (please google this), weight loss and certain medications such as Colchicine (during flareups) and Allopurinol (to prevent flareups). You should have your uric acid level checked.

Rotator cuff: If you can't move the arm above the shoulder you probably have a rotator cuff syndrome and if you have numbness and tingling you have nerve compression- these two may or may not be related. You should see your pcp or orthopedics.

It : It really depends on the facility. In general bigger hospitals charge more so you should look for smaller clinics. I do spirometry/pfts in my office for about $50 without insurance.
