Why save 3rd molar?: #32 is not easy to reach, difficult to clean, and extraordinary hard to endodontically treat successfully. There must be extenuating circumstances for you to try (this hard) to keep a tooth that most people have removed in their teen years? But, if a re-treatment is called for- then my one suggestion is to defer to an endodontist. If a specialist tells you it is worth repairing, then proceed.

Not very likely: A rct that has failed on a third molar (32) is unlikely to be successful a second time. Treatment by an endodontist (specialist in root canals) will improve the odds, but likely not by very much.

Depends: Depends on the reason for failure and if a top endodontist did the original endo. We have done root canals on many wisdom teeth and they can be tricky due to there amorphous root forms. That being said, if they are done originally by a top microscopic endodontist, it can be done successfully. Often times the first root canan may miss canals or ledge curved canals making retreatment difficult. Have.

Unknown: Root canal therapy can be quite challenging on tooth #32. Between the internal anatomy and restricted access, success is not always possible. Sometimes extraction may be a better alternative. Discuss your questions with your endodontist or general dentist to see which might be best for you.

Unknown: Tooth #32 is a lower right wisdom tooth. With a failed rct it is a time to consider the concept of diminshing returns. How much time, money, discomfort are you willing to expend on a wisdom tooth? Only you and your dentist can answer that question. Please consider all of your options.
