Swallowing: Painful swallowing is called odynophagia, usually related to heartburn, but could be yeast infection in the esophagus. Upper back area is somewhat unusual. Chest X-ray would be helpful.

GI tract: Most likely you have a problem with your digestive system, problems with esophagus, stomach, duodenum, pancreas and gall bladder are common sources. Neurologic causes of this problem are unlikely.
