Spasm: Discomfort possibly due to spasm in the esophagus although usually discomfort is felt in the chest & not the back. See your doctor.

See your doctor.: Could be gastro-esophageal reflux, or peptic ulcer disease. This is not associated with the usual muscle strain symptoms if it hurts just when you swallow.
