Would you personally, or do many doctors allow their patients to start suboxone if they're currently at 40mg of methadone almost daily?

Depends: If you can go 2-3 days without methadone, to the point where you are feeling at least mild withdrawal, then Suboxone is usually tolerated, but i would go to a prescribing physician who has experience with this. I usually give patients transferring an initial very low dose (1-2mg) rather than 4 mg to make sure it doesn't make withdrawal worse.

No: You need to stop Methadone for several days and be in full withdraw for Suboxone induction. Mixing Methadone and Suboxone would cause withdrawal. The dose for Suboxone will be determined by your doctor during the induction. Please see a doctor asap and discuss your need.
