Sometimes: Depending on cyst size & rate of change of size, pain can be one-sided and intense, and physical exam can suggest para-ovarian / proximal fallopian issue. Yes, ultrasound is the way to distinguish in most cases.

Essentially you can: Distinguish the two, but could depend on how well you see it. Any time you are pregnant and do not have an intrauterine pregnancy, then ectopic cannot be excluded. The next thing you look for is a mass outside the ovary. In general, it is pretty easy to tell if it is a paraovarian cyst or something more ominous, but certainly not 100% of the time.
