Very concerned: You have lost 2/3 rd of your kidney function or capacity. Should be on 1gm/ kg/ day protein restriction, on renal vasodilating medications for blood pressure. Avoid nephrotoxic meds.

VERY CONCERNED: If this is a new problem, you should be concerned and see a kidney specialist. It is possible that you have had this for a while and the cause could be diabetes, still would see kidney doctor to find the cause of kidney problem and come up with a treatment plan.

GFR of 37 ml/min: You are quite fortunate that for a woman with 53 years of diabetes, who is a type 1 who developed it at 10 years of age, now 63. You have stage 3 chronic kidney disease (ckd). You had excellent glucose and blood pressure control to have such a good level of ckd. If you not have one, get a nephrologist to follow up as you may need some treatment soon for anemia of CKD and hyperparathyroidism.
