IV sedation is opt: Typically IV sedation is optional. It depends on your anxiety level or surgery complexity. Many wisdom teeth have been removed under local. Consult your dentist if you don't want it and if he insist and you still are unsure get a couple more opinions.

Depends: This depends on several factors. How difficult will the extractions be. Does the patient have a high anxiety level. It all depends on the comfort level of the patient. I have taken out wisdom teeth that were very difficult on some patients and they were fine with just local anesthetic. Other patients need very heavy sedation. Find a dentist or oral surgeon that is willing to discuss this with you.
