Two approaches: There are two approaches to an epidural injection. The midline approach spreads steroids in a space that often spreads to the nerve roots at one or more levels. The lateral approach (transforaminal) bathes the medication over a specific nerve root. There may be some spread to other nerve roots as well. This approach requires x-ray guidance for optimal effectiveness.

No: The medications in the injection will be placed in the epidural space near to the affected nerve root. The medication will bathe that nerve root, but not be directly sprayed on the nerve.

Transforaminal Inj: In the transforaminal approach to the epidural steroid injection the nerve is coated with the steroid/anesthetic mixture.

Not directly,: that's why it's called epidural. But some does leak out of holes in the epidural space to reach the nerves
