Usually: The increase in growth hormone pulses drives growth while the increase in gonadotropin drives acne. One may start before the other -- think of the very tall 16 year old boy who doesn't have his pubic hair, or a boy with very early acne and puberty whose epiphyses are closed by sex hormone before he can gain his genetic height.
