Heart attack: If you have a heart attack yes you will need angiography regardless of your test results.

Exercise test: I don't know how your heart attack was diagnosed. Anyway, you will need a exercise test or perfusion scan with Dipyridamole or exercise echo. Main purpose is to show you don't have inducible/provocable ischemia.
