Many things: There are a number of potential causes such as infection , hormonal changes, or even endometriosis. I would get another exam and possibly a second opinion.

Bleeding with clots between periods. Heavier than spotting. Clear pelvic ultrasound, normal ovaries, no free fluid. HPV negative. Normal pelvic exam. Slightly elevated Ca125 of 50. Not on medication. Cancer?
