See headache spec.: Doubtful you have had all the potential therapies, but consider, if not yet tried, break up pattern with steroids, depakote, topirimate, naprosyn, (naproxen) indocin, lithium, botox. Acupuncture, occipital nerve blocks, surgical denervation procedure, or try otc, riboflavin, coq-10, butterbur, feverfew. See headache specialist, and get a new approach.
