Need more info: I'm assuming you have a tubo-ovarian abscess but i can't be sure unless you give me more information. Regarding surgical infections, in general, if your pain and fever return after finishing a course of antibiotics, you likely have a source of infection that would require drainage / removal. You need to see your doctor to be evaluated for this.
