Dentist&orthodontist: Although 11 is behind the age curve for incisor eruption, many factors can cause delays. A dental exam and x-rays are needed for diagnosis. And a consultation with an orthodintist is needed for treatment planning. A surgical opening will only succeed if there is enough space for the teeth to erupt. And access is needed to reach an impacted tooth. Consult a dentist to get everyone involved.

Ankylosis: 'multiple surgical opening' should have enabled a non-erupted tooth to erupt...Unless the tooth is ankylosed-which means fused to the bone that surrounds it. Thorough evaluation is necessary by a dentist/oral surgeon or orthodontist to determine the diagnosis/prognosis.
