I would: Any injury substantial enough to avulse the nail may also have caused injury to the nailbed or fracture of the toe bone. A podiatrist can evaluate the injury, check for fracture or infection and recommend local care to speed healing. The fact that the injury was months ago may limit your options, but its always wise to have it checked.

Yes: It is possible you have an infection or underling fracture. If the nail bed itself was lacerated, your tetanus history should be reviewed and you should be placed on an antibiotic.
